Asthma Review Project
Service Goal:
To identify people who need an asthma review, have had exacerbations or there is an indication their asthma is poorly controlled (overuse of SABA inhaler) and signpost to community pharmacy to have their annual review with the pharmacist.
How:
- Patients within the eligibility criteria will be identified in the general practice and passed to the nominated community pharmacy. The patient will be offered an asthma review which meets QoF requirements in the community pharmacy including:•inhaler technique
- •assessment of asthma control using ACT score which includes assessment of SABA use
- •patient-reported record of exacerbations
- •check that the patient has a personalised action plan in place

- To identify the ~17675 people in Bury with undiagnosed Hypertension
- To identify the ~834 people in Bury with undiagnosed Atrial Fibrillation

- Referral & Signposting
Service Overview:
North Manchester Hypertension Project
Service Goal:
- To identify people with undiagnosed Hypertension in North Manchester
- To identify people with undiagnosed Atrial Fibrillation in North Manchester
How:
- Opportunistic brief interventions with BP & Pulse measurement in community pharmacy
- Structured lifestyle consultations with those who have raised BP
- Post-diagnosis support through NMS for those ‘found’ through the service
