Community Pharmacy providers across Greater Manchester can now access a patient’s full medical record via the GM Care Record via PharmOutcomes, improving the quality and safety of care provided to patients in the community.

How can Community Pharmacists access the GM Care Record?

There are 3 simple steps to follow to gain access to the GM Care Record:

Step 1

• Supply the IP address for each pharmacy to ensure that each pharmacy’s IP address is not blacklisted for access.
  

• Click here to submit your IP address

• If you are unsure of your IP address, you can find it in Windows:

  • Select Start > Settings > Network & Internet> Wi-Fi and then select the Wi-Fi network you’re connected to.
    

  • Under Properties, look for your IP address listed next to the IPv4 address.

Step 2

• Please have your Data Protection Officer complete the onboarding form to ensure compliance with data management requirements.

Step 3

• Your Data Protection Officer will also need to download and sign the Data Sharing Agreement, then email it to the Information Governance team at IG@healthinnovationmanchester.com.
